The following questions must be answered in order to assign an emergency exit seat. Please be advised that the final determination for acceptance will be made at the airport and, if it is determined that you do not qualify for emergency exit seating, you will be moved and you may or may not be seated together. If you answer Yes to any of the following questions, you are not considered qualified for emergency exit seating. Please take other seats. This form must be submitted before emergency exit seats can be assigned.

 Do you lack visual capacity beyond eye-glasses or contact lenses?

 Do you have a condition that might prevent you from performing evacuation functions?
 Are you unwilling to assist in an evacuation?
 Do you lack mobility, strength or dexterity in both arms, both hands, or both legs to:
 quickly reach the exit?
 remove obstructions?
 manipulate the exit door and slide mechanisms or lift out the window exit door?
 exit the aircraft?
 assist others in exiting?

TSA Privacy Notice

The Transportation Security Administration of the U.S. Department of Homeland Security requires Vacations-Hawaii to collect information from you for purposes of watch list screening, under the authority of 49 U.S.C. section 114, and the Intelligence Reform and Terrorism Prevention Act of 2004.
